Do you want to make a real difference in the lives of Aboriginal children and young people?
We are seeking a committed and culturally responsive person to take on the role of Aboriginal Child & Youth - Health Pathways Navigator. You will support Aboriginal children and young people in out of home care to access timely, culturally safe, health care and wellbeing support
This role is about more than coordination — it’s about wrap around care, advocacy, cultural connection and walking alongside children, families and carers
In this role, you will:
- Identify and respond to unmet health and wellbeing needs of Aboriginal children in care
- Coordinate culturally safe health assessments, including Aboriginal health checks and screening programs
- Develop and review culturally appropriate care plans alongside children, families and carers
- Support families to access services and navigate complex care systems
- Liaise with Child Protection, carers and service partners
- Maintain accurate client and program documentation
We are looking for someone who brings both professional capability and heart to their work. You will have:
- A relevant qualification and registration (RN/EN, Social Worker, Aboriginal Health Practitioner or similar)
- An understanding of clinical and holistic care services and processes.
- Experience or knowledge of child protection and/or out of home care
- The ability to develop client-centred care plans and coordinate service pathways
- Strong relationship-building, advocacy and communication skills
- Cultural awareness and a strengths-based approach to care
