Join Our Team as a Health Navigator – Pathway to Good Health
Albury Wodonga Aboriginal Health Service (AWAHS) is excited to announce a rare opportunity to join our innovative Pathway to Good Health (PTGH) program. We are seeking a passionate Health Navigator to deliver child and young person-centred care, supporting Aboriginal children in out-of-home care to access timely, culturally safe health assessments, referrals, and wrap-around care.
About the Role
- Salary: Up to $72,000 per annum, commensurate with experience
- Employment Type: Full-time, fixed-term until 30 June 2026 (with potential for extension)
- Location: Based in Albury, NSW, with work across the border in Victoria
As a Health Navigator, you will:
- Collaborate with PTGH team members, VACCA, Dept Communities and Justice, carers, and service providers to deliver quality, culturally sensitive care plans.
- Advocate for families, ensuring respectful and inclusive service delivery.
- Act as a critical bridge between families, carers, and health systems to empower children and young people.
- Combine direct service provision with strategic service planning and administration.

Who We’re Looking For
Our ideal candidate will have:
- Qualifications: Registered or Enrolled Nurse, Social Worker, Aboriginal Health Practitioner, or other recognised Allied Health professional with AHPRA registration (or equivalent).
- Experience: Working with children and young people (0-17) and knowledge of child protection or out-of-home care systems.
- Skills: Ability to build trust, advocate for families, coordinate care plans, and navigate local health systems and referral pathways.
- Passion: A commitment to holistic care, improving health literacy, and enhancing health and wellbeing for children and families
- Teams: Sharing your experiences and knowledge with multidisciplinary allied and primary health teams
- Requirements: Current Working with Children Check, Driver’s Licence, and willingness to obtain a satisfactory Police Check.
