You will be involved in a variety of family medicine including triage, health assessments, chronic disease care, preventative care and supporting patients through their healthcare journey. Working alongside GPs, nurses and a supportive multidisciplinary team, you will play an important role in care planning, referrals, procedures, wound care and health promotion

The Role

We are looking for a motivated and community-focused Practice Nurse (RN) to join our team full-time for a 12-month contract, with the potential to extend.

In this role you will:

  • Be the first point of support for clients, coordinating urgent GP appointments and managing waitlists with empathy and professionalism
  • Work collaboratively with GPs, allied health and external providers to ensure clients receive seamless, quality care
  • Provide hands-on nursing support including wound care, diagnostic procedures, chronic disease management and health assessments
  • Assist with medication management and initial emergency presentations
  • Support health promotion and preventative programs across all ages, from children to Elders

What We Are Looking For

We would love to hear from you if you are:

·  An AHPRA Registered Nurse with a minimum of 2 years post graduate experience

·  Skilled in wound management, chronic disease care and health assessments

·  Accredited Nurse Immuniser- desirable but not essential

·  Passionate about health promotion, prevention and community wellbeing

·  Able to thrive in a fast-paced, team environment where collaboration matters

·  Strong interpersonal skills, integrity and excellent time management

·  Understanding of health issues impacting Aboriginal people
